The personal information on this form is collected by Cypress View Foundation under section 33(c) of the Protection of Privacy Act (replacing the former FOIP Act as of June 11, 2025) for the purpose of administering applications for subsidized housing or rental benefits. I authorize Cypress View Foundation to make any inquiries to verify the facts contained herein by any method deemed necessary; being fully aware that discovery of any false statement shall cancel any further consideration of my application.

CONSENT TO REFERENCE CHECK

I hereby consent to the Cypress View Foundation doing a reference check with my current and previous landlords. I understand that personal information will be collected concerning my previous tenancies including, but not limited to, any problems that may have occurred including non-payment of rent, problems with other tenants, and damage done to landlord property. I understand that the purpose of the collection of this information will be to determine my acceptability for residency at Cypress View Foundation.

RESIDENT DISCHARGE/ EVICTION
1. Residents may be discharged when they are no longer appropriate for accommodation because they:
• Are unable to care for personal needs and medications within the services provided by Home Care and the services of the Foundation.
• Are unable to manage incontinence problems within the services provided by Home Care and the Foundation so that the incontinence does not negatively affect the lives of other residents or the Foundation operation.
• Are unable to get back and forth to their room when needed, within the services provided by Home Care and the CVF
• Require a special diet that the lodge is unable to accommodate.
• Display behavior problems that may make them a threat/risk to other residents, staff, or themselves, which negatively affect the lives of other residents or the Foundation operation or which may require specially qualified staff.
• Have alcohol or drug problems that negatively affect the Foundation operation or the lives of other residents.

Management shall work with the resident, family members, and appropriate community services to move the resident to appropriate accommodation.
2. Residents may be asked to move out of the facility for the following reasons:
• Performs illegal acts or carries on an illegal trade, business, or occupation in the facility or property
• Endangers persons or property in the facility or property of the CVF.
• Does or permits significant damage in the facility or property of the CVF.
• Does not maintain their unit in a reasonably clean condition.
• Theft, dishonesty, or destructive behavior.
• Failure to abide by any Foundation policy.
• Failure to pay rent.

Residents may be asked to move out within 24 hours for the following reasons:
• Does or permits significant damage to the facility or property of the CVF.
• Physically assaults or threatens to assault the landlord, another resident or visitor.

All discharges shall be reviewed with the CAO prior to proceeding.

RELEASE OF OBLIGATION ACKNOWLEDGEMENT
I understand that this application does not constitute an agreement on the part of the Cypress View Foundation to provide me with rental accommodation.

I further acknowledge the right of the Cypress View Foundation at any time before the execution and delivery of a lease hereby applied for, to withdraw, revoke, or cancel without penalty or liability for damage otherwise, any acceptance or approval of this application previously made or given.

a) Cypress View Foundation to verify all information relating to this application and any future information provided throughout the entire tenancy period. This may include but is not limited to employers, credit bureaus, financial institutions, federal, provincial, or municipal government department, City of Medicine Hat Utility Department, offices, agencies, boards, or landlords

b) Cypress View Foundation to release and exchange any information and documents including personal information by and between Cypress View Foundation and such other authorities as, but not limited to all federal, provincial, and municipal departments or offices, social support agencies, interpreter(s), credit bureaus, financial institutions or past or current employers.

c) The parties/agencies noted in the previous paragraph are to release the same such information to the Cypress View Foundation.

d) Cypress View Foundation to obtain information from any person or agency for audit or verification of my income or circumstances.

Cypress View Foundation is authorized to collect this information under Part 2, Division 1, Section 33, © of the Freedom of Information and Protection of Privacy Act.

CONSENT TO RELEASE OF INFORMATION

Many organizations that aid and/or benefit will not release information without written consent from the recipient. Cypress View Foundation requires the following to be signed by all applicants:

a) Cypress View Foundation to verify all information relating to this application and any future information provided throughout the entire tenancy period. This may include but is not limited to employers, credit bureaus, financial institutions, federal, provincial, or municipal government department, City of Medicine Hat Utility Department, offices, agencies, boards, or landlords

b) Cypress View Foundation to release and exchange any information and documents including personal information by and between Cypress View Foundation and such other authorities as, but not limited to all federal, provincial, and municipal departments or offices, social support agencies, interpreter(s), credit bureaus, financial institutions or past or current employers.

c) The parties/agencies noted in the previous paragraph are to release the same such information to the Cypress View Foundation.
